Two different moderators gave him infractions for different reasons, neither with the intent to ban him at the time.  Come to find out, our combined infractions was enough to "mute" him, which is a function I didn't know existed until it happened.  Strangely, I find more satisfaction out of having him muted right now than banned.

There is no option to approve his posts.  They are just muted.

Pretty much this happened. But yes, warnings are usually only given once. Then a ban should follow.

The final warning he received was for spamming the same two phrases over and over in a post.
